My late husband had several items embroidered at Designer's Top Shop. They have a nice little shop in downtown Avon Park not too far off Main Street. They made a couple of embroidered patches and baseball caps for my husband. The woman who owns the shop is very helpful in designing the item you're interested in and her work is very good, but their turn around time has always taken longer than first promised.
Designer's Top Shop sells shirts, caps, jackets, shorts and a few other items in their store (although their prices seem high to me). They will either screen print or embroider a design on these items or something you've brought in.
