Fountain View Care Center is a Medicare and Medicaid certified skilled nursing facility that offers short-term rehab and extended care services. We believe in maintaining the dignity of each resident through a personalized team approach. We promote the health and well being of all of our residents, recognizing that every individual has physical, social, and spiritual needs. Our professional staff provide continuous care and evaluation to help residents meet these needs and to maintain the highest level of independence possible. Each resident has a plan of care that is individualized based on the consultation and input of the resident, family members, physician, nurses, social worker, dietary manager, activity director, and therapists. Invitations are sent quarterly to encourage ongoing family participation in care planning.
