(if not P.P.O. there is a $100 exam fee),A.S.H., P.I.P., -Workers' Comp. (you must have the claim # available at time of booking) -any Copays/coinsurance/deductibles apply to all policies -Medicaid is not accepted - We do not perform work or school physical exams. We treat a variety of conditions including but not limited to; car accidents, sports injuries, work related and compensation injuries, headaches, back pain, and extremity pain. We also provide supplement support with vitamins, minerals and herbals, but we are not a dietitian nor a nutritionist, nor a Physiotherapist. We do not accept Medicaid.
