*If You Have Insurance Benefits, Please Bring The Necessary Forms And Information To Your First Appointment. As A Courtesy To You We Will Submit All Of The Charges To Your Insurance Company. Deductibles And Co-Payments Are Due On The Day Service Is Provided. Please Remember That The Financial Obligation For The Dental Treatment Is Between You And This Office. The Insurance Company Is Responsible To You And Not This Office. All Accounts 60 Days Since Th e Date of Service Will Be Charged 1 1/2% Of The Unpaid Balance (18% Per Annum.)
