MAP is a Federally Qualified Health Center that provides medical care to everyone in the community. All forms of insurance, as well as Medicaid and Medicare, are accepted. MAP has a sliding fee scale program to help individuals without health insurance. The sliding fee scale program is based on income. Those who are approved for the program will recieve a discount on healthcare services. Services can cost as low as $25. MAP will continue to include the current services of: Physician Services with a focus on Chronic Care Management for diabetes, hypertension, obesity and asthma; Screenings for various forms of cancer; dental services; and pharmacy assistance. Enhanced services includes OB/GYN services; expanded dental services through partnership with Christ Community; and more focused approach to community outreach. A particular emphasis will be directed towards assisting patients with medication adherence. Screenings will be schedule monthly to address health awareness issues a nd promote opening of the new facility. MAP also serves as a vital resource on linking patients without health insurance to the health exchanges with education sessions.
