Lake Park is a more than 250-bed long-term skilled nursing care facility located in Sylvania, Ohio. It is a member of ProMedica Health System and is certified by the Ohio Department of Health to meet Medicare, skilled Medicaid and participation requirements. Its rehabilitation services include physical, speech, occupational, recreational and respiratory therapies. The organization s staff includes social workers experienced in geriatric care and a chaplain who provides guidance and counseling services. Other services provided by Lake Park include respite care, end-of-life palliative care through hospice unit, Alzheimer s and dementia care, gift shop, beauty salon and barbershop. Lake Park s nursing team works in coordination with nurses, therapists, physicians and social workers at the Flower Rehabilitation Center in Ohio.. Lake Park offers the area's most extensive rehabilitation program including:

Lake Park also offers Easy Street,the only simulated environment of its kind in the region which enables patients to gain strength and practice the functional skills they will need when returning home. Memory Care is an 18-bed unit designed to meet the needs of residents requiring Alzheimer's or dementia care. The home-like atmosphere encourages individuals to function at the highest level possible by focusing on each patient's strength and abilities in a safe,comfortable environment.
Comfort Care is a 17-bed palliative care unit providing compassionate care to those facing life-limiting illness. Our team focuses on maintaining the highest quality of life for each patient.
