In Colorado, there is a growing need for older adult service organizations at the community level to offer "one stop shopping" to concentrate programs in a manageable, accessible and understandable way. In 2006, SRC launched its new Coordinated Care Model to address fragmentation of long-term care services, and to more effectively engage with complex regional services that may often require the intervention of a trained, professional care navigator to ensure the accessibility of the fullest possible range of community services for the benefit of older adults and caregivers.
SRC believes that older adults ought to be able to call as few numbers as possible in order to access resources, information and services. SRC analyzed the core program areas that it offers, and defined the services that clients may need, but that it does not have the existing capacity or expertise to provide directly.
SRC then sought to identify partners to collaborate with to make seamless community-based care a reality in the Denver metro region by linking its individual services to other providers of complimentary services on behalf of older adults and their caregivers. The result has been a series of partnerships that have engaged corporate, health care, public sector, not-for-profit and philanthropic resources in ways that are new not only to Colorado but that also have served as nationwide models..
