I made an appt with Center for Women's Health upon the discovery that I was pregnant with Lisa Gayle. She is the nurse there and they said i was required to see her first. I made the appt several weeks ago. So I get a message that was partially cut off from them the day before confirming my appt. The message stated that I needed to fill out a patient history form. None of this info was told to me when i made the appt and by the time i got home from work to fool with it they were closed. They did say at the time of appt that I would get a packet in the mail that never came. Anyways i got in early to the appt and waited at the window to tell the lady that i didnt see a history form online. I did however print out a bunch of forms online and filled out a patient information paper. She noticed i was waiting and told me to have a seat and she would get back with me. i sat there 15-20 min while she had the glass closed and saw no one before she called me and realized my appt was at their other location on the same street right next to it. So i rushed over there, still early for my appt, and told the girl that the front desk the situation and she went and spoke with Lisa the RN and she said she wouldnt see me since i didnt fill out the history form. I did fill out my information form, the only form i could find online. Thanks Lisa! First off, I have never heard of it being a requirement that you have to fill out everything online? Do you people own a printer that you can print it, and you could give it me to fill it out, wouldnt we go over my history together inthe office? I took of work to come there, isnt my time just as important? I was there early and your company failed me. Then I was told the next appt available was in Feb...its Jan 11! I refuse to let them reschedule anything. That is absolutely rude, and not a way to treat paitents. i sure dont want to deal with them the entire pregnancy...not to mention have them deliver my child. they should change this practice. VERY Disappointed.
