After my medical insurance went up to over $400 a month, I checked out Benco. They do Blue Cross/Blue Shield of Florida.
I got a policy with them and it was loaded. A few years later I went for an H.S.A. account with them. They seemed to take ages and didn't return my phone calls.
In the end I finally got the plan I wanted and saved a bunch of money.
