A quick call to the B-ham Business Bureau names the owner as Andy Andrews...I believe he is the son of Doctor Andrews of former HealthSouth fame (now at St Vincents).
It is very possible that the membership was bonded, meaning there is a slim chance of getting your cash back, or at least a portion. I do not know the process, but a filed lien may be the way to go.
