can't be bothered to introduce himself

I've never had an invasive procedure (colonoscopy) where the doctor didn't introduce himself. I have no idea what he looks like. I had several questions after the procedure and he still didn't bother making an appearance. Instead, I was given a "report form" with some items circled - as if that explained everything. If you're interested in a healthcare partner go elsewhere.
