Rebates range from $30 to $40 and entail purchasing (4), (8) or (24) boxes of a variety of different lenses. You must get an eye exam and purchase the product within 90 day of the eye exam. Complete the MIR and attach your ORIGINAL receipt from the purchase of the lenses, as well as a COPY of the receipt from your eye exam. Also include UPC codes from half of your product purchase.
