Welcome to Care Center of Rossmoor. On behalf of our entire team, we thank you for considering our transitional care and sub acute center for you or your family member's care.


Our wide variety of services enable the patient to receive the medical care they need, the restorative therapy they require, sub acute services for patients with tracheostomies, including those who require mechanical ventilation, and all the support they and their families deserve. We serve many types of patients with a wide variety of needs whose ages range from 18 to over 100 years

From intensive short-stay rehab to longer-term restorative care, our nursing and rehabilitation centers provide a full range of medical and social services to treat and support each of our patients and residents. Many of our residents return home, but for those who cannot, we provide safe, compassionate care in an environment that fosters independence and dignity.


A Bridge between Hospital and Home


When patients leave the hospital, they often need continued care to recover completely. That¿s where we come in. We work with you to set a care plan to meet your goals and abilities. Patients typically receive up to three hours of intensive rehabilitation each day, provided by our specially trained interdisciplinary team consisting of:
